Navigating the new IPP Person-Centered Plan without excessive burden

The new IPP person-centered plan (PCP) requirement adds another layer of complexity to case loads. Case managers are now responsible for ensuring every case aligns with a personalized, detailed plan—similar to NeuroNav’s existing person-centered planning approach.

Meet Justin

Justin is 29-year-old with learning disabilities and seizures. Despite these challenges, he is determined, creative, and social. He discovered a passion for dance, theater, and rap classes—activities that bring out his personality, boost his confidence, and connect him with friends. Through SDP he has gained independence, moved into his own apartment, and built a vibrant life full of confidence, friends, and meaningful work.

Meet Phillip

Phillip is a curious, energetic 16-year-old on the autism spectrum who loves sports, outdoor adventures, and spending time with friends. Through SDP, he’s gaining independence, confidence, and meaningful community of social connections.

Meet Logan

Logan is a 28-year-old with Down syndrome and epilepsy. Logan is easygoing, enthusiastic, and loves being outdoors, and through SDP, he’s built a fulfilling life of horseback riding, swimming, volunteering, and community connections.

Meet Ashley, a 33-year-old who sings in the Oakland Interfaith Gospel Choir and is passionate about all things music. Through SDP she's able to continue attending her day program part-time while gaining access to a Braille instructor and taking online classes at a local community college.
