Writing a Person-Centered Plan That Works for You

 April 23, 2026 at 12pm

You know your life better than anyone, and your Person-Centered Plan is where that knowledge becomes your most powerful tool in the Self-Determination Program.

For many people in the Self-Determination Program (SDP), the Person-Centered Plan (PCP) becomes routine paperwork: updated on schedule, filed away, and forgotten until next year. But when it's done well, the PCP is one of the most useful tools in your self-determination journey. It shapes your goals, drives your budget, and puts your voice at the center of every support decision. This webinar is a chance to step back and look at the PCP with fresh eyes.

If you're already in SDP: we'll give you practical tools to refresh your plan so it reflects who you are right now and where you want to go, whether you're heading into an annual update or your current plan just doesn't feel right anymore.

If you're considering Self-Determination: we'll walk through what makes a PCP genuinely person-centered and why getting it right from the start makes everything else easier.

What You’ll Learn

  • What separates a powerful PCP from one that just meets compliance requirements
  • How to audit and refresh an existing plan, or where to start if you're building one for the first time
  • How to use the past year's data to inform stronger goals for the year ahead
  • How to write goals that are specific, motivating, and written in your own voice
  • How your PCP connects to your budget, and how a well-written plan strengthens your position in planning conversations

Who Should Attend

This webinar is for individuals and families who want to get more out of the Person-Centered Plan, whether you're already in Self-Determination or still exploring whether it's the right fit.

If you're currently enrolled in SDP, this session will help you strengthen and refresh a plan that truly reflects where you are now and where you're headed.

If you're considering Self-Determination, it's a chance to understand how the PCP works and why it's the foundation of everything in a self-directed support system.

Your plan should sound like you, work for you, and move you forward. This session will show you how to make that happen.
